Providence Transitional Care Centre - PTCC 64 bed inpatient Reactivation Care Centre * this model supports individuals who no longer need acute care but are not yet ready to go home * two units are for patients in a high level of activation mirroring their routines at home * also includes key support services, with spiritual care, social work, discharge planning and home and community care * stays of 60-90 days * funded by the Ministry of Health
