Program: Lung Health Services

Description of Services: In partnership with ConnectWell Community Health, our Lung Health Program offers support for people living with lung disease. Our programming includes:
  • Assessment
  • Spirometry (breathing test)
  • Developing a client-centered action plan
  • Follow-up care for clients with chronic lung disease (COPD, asthma, pulmonary fibrosis)
How to access the program: This program requires a referral from your primary care provider. When you are referred to the lung health program, you will be seen by a Registered Respiratory Therapist/Certified Respiratory Educator (CRE). The CRE will help you understand and manage your symptoms.
Together, we’ll come up with a plan to help you navigate through your symptoms.
